Student Forms

Over the summer, each student will receive a packet of forms to be completed by the indicated deadline. Returning students are invited back to the program and acknowledge their return in the Scholarship Renewal form. Each student must read the Student Handbook and acknowledge that they understand the program guidelines. Medical forms include Insurance and Emergency Contact Information and Authorization for Consent to Medical Treatment with insurance information and medical history. The Authorization for Consent to Medical Treatment form gives authority to Andover ABC, the Resident Director, and host families, to obtain medical treatment for the student while she is living in Andover. The Travel Permission and Photo Permission forms convey the parent/guardians' preferences for student travel and use of photographs.

These completed forms are distributed to the host families. The medical forms must be signed and mailed back, or delivered in person, before the start of the school year. Andover High School also requires medical forms, which must be returned before the first day of school.
